Stroke – follow-up care

Coordinating ongoing care after a stroke.

Understanding the condition

A stroke results from impaired blood flow or bleeding in the brain. Difficulties with movement, speech or other abilities may remain after acute treatment. GP follow-up complements neurological care and rehabilitation.

Common symptoms

New weakness on one side, a drooping mouth, sudden speech changes or visual problems may indicate another stroke. Immediate assessment is necessary even if symptoms quickly disappear. Call 112 rather than waiting for a practice appointment.

Diagnosis and assessment

We review discharge reports, the presumed cause and remaining limitations. Blood pressure, lipids and other vascular risks are checked. Heart rhythm problems and other possible causes are followed up according to the specialist plan.

Treatment and ongoing care

Follow-up includes safe medication use, managing risk factors and coordinating therapies and practical support. Physiotherapy, occupational therapy or speech therapy may be important. We support rehabilitation, care questions and return to everyday life with the professionals involved.

Monitoring your progress together

We discuss which changes to watch for in everyday life and when another check-up would be useful. Your questions, personal goals and how well you tolerate treatment inform further decisions. Where needed, we coordinate with your treating specialists.

Your appointment at the practice

Bring your health insurance card, recent medical letters, existing test results and a medication list. Note any changes in your symptoms and your main questions. Do not change existing treatment yourself; we will discuss together whether adjustments are needed.
