We provide GP-led palliative care for people with a serious, incurable illness. Your wishes, quality of life and support for your family are at the centre of this care.

How we support you

We discuss distressing symptoms such as pain, breathlessness or nausea and tailor treatment to your personal situation. Together with you and your family, we plan the next steps and coordinate care with nursing services and other clinicians.

Working with specialist community palliative care

When symptoms are particularly complex and specialist support is needed, we work with a specialist community palliative care team (SAPV). The team complements GP care with specialist medical and nursing expertise. We assess eligibility individually and help arrange the prescription and coordination.
